This attachment checklist completes the Clean Ohio AEPP online application. Attach the following items to the printed online application for submission. The application may be deemed incomplete if applicable attachments are not submitted by the application due date. Please label all attachments clearly by the designated attachment letter.

In addition to submitting the application electronically, all applications must be printed out and submitted by hard copy with signatures and attachments. To do this, applications can be sent by registered or certified mail to our office or hand delivered on or before 5:00 p.m. on Friday, May 23, 2008.

ATTACHMENT A: Resolution(s)

If the sponsor is a local government:

Attach a resolution from the local governing body that:

  • Supports the application,

  • Confirms either a minimum 25% monetary match or the landowner's minimum 25% donation of the "Points Based" appraised value.

  • Commits to monitor and enforce the easement in perpetuity

  • If the sponsor is a township or county, a copy of this resolution must be sent to the other local governing body (township or county) to give it an opportunity to comment upon the sponsor's resolution.

If the sponsor is a non-profit organization:

Attach a resolution or recorded action from the non-profit organization that:

  • Supports the application,

  • Confirms either a minimum 25% monetary match or the landowner's minimum 25% donation of appraised value; and

  • Commits to monitor and enforce the easement in perpetuity.

In addition, attach a resolution or recorded action from at least one local governing body(township or county) that:

  • Supports the application,

  • A copy of this resolution must be sent to the other local governing body or bodies(the county or township(s)) to give it/them an opportunity to comment upon the resolution.

ATTACHMENT B: Plat Map

An 8 1/2 x 11 inch plat map or other map showing the name of the landowner(s) and the parcel number(s) of the entire farm, with the homestead delineated (black lines) and the homestead perimeter clearly indicated in feet.

ATTACHMENT C: Tax Records

County Auditor tax records of the applicant farm's land values used in responding to Step 7 of this application e.g., the real estate taxes and assessments (tax bill), or other real property records.

ATTACHMENT D: Map of the Applicant Farm

An 8 1/2 by 11 inch map, sketch or aerial photo which shows all information claimed in Steps 12 and 13. This includes, but is not limited to the proximity of the application farm from any protected properties(easement and non-easement), to neighboring applicant farms, sewer and water lines, accessible public roadway intersection(s), and the total road frontage of the farm.

ATTACHMENT E: Soils Map

An 8 1/2 by 11 inch soils map and soils legend with the farm's outer boundary outlined.

ATTACHMENT F: Historical Designation

For land or structures that have been historically designated by the Ohio Historic Preservation Office, attach written verification from the Ohio Historic Preservation Office confirming that the land is historical and when it was designated as such.

ATTACHMENT G (if applicable): Corporations, Partnerships, and Trusts
  • A copy of incorporation papers, and

  • Certificate of authorization that the individual is authorized to act on behalf of the Corporation, Partnership, or Trust.

ATTACHMENT H (if applicable): Non-Profit Organizations

The Ohio Department of Agriculture may ask a non-profit organization for the following additional information:

  • IRS tax exemption,

  • A copy of by-laws showing that the non-profit organization's mission includes farmland preservation,

  • A list of officers, board of directors, and members,

  • Financial statements,

  • Stewardship endowment policy, and

  • Documentation showing the non-profit organization's ability to hold, monitor and enforce an agricultural easement in perpetuity.
