After the sample has been collected
according to the ADDL's guidelines, fill out the
ADDL Sample Submission
Form.
Once the form is filled out completely,
the form and sample can be submitted to ADDL by mail, courier, or
dropped off at the lab.

Guidelines
Please submit the appropriate specimen for the desired test. Refer to
the various laboratory sections for information concerning specimen
collection and shipment. Federal regulations govern the packaging and
labeling of diagnostic samples. Many couriers refuse to handle packages
that are leaking or show signs of internal breakage.
Instructions:
Ship specimens in appropriate leak-proof containers.
Screw-cap closures must be reinforced with tape and/or double
enclosed.
Label each specimen container with owner's last name, animal
name/number and type of specimen.
Double bag specimens shipped in whirl-pak bags.
Add absorbent material to prevent leakage should breakage occur.
