How do I get forms?

Regulatory forms can be requested by a licensed and accredited veterinarian three ways:

What is the latest time to drop off a Coggins test and be able to receive results the next day?

If the test is submitted by 2:30 pm, the results will be available by noon the following work day.

What are the lab hours?

Monday - Friday.

8:00 am to 5:00 pm. 

Excluding the following holidays:

  • January 1 - New Years Day

  • Third Monday in January - Martin Luther King Day

  • Third Monday in February - President's Day

  • Last Monday in May - Memorial Day

  • July 4 - Independence Day

  • First Monday in September - Labor Day

  • Second Monday in October - Columbus Day

  • November 11 - Veteran's Day

  • Fourth Thursday in November - Thanksgiving

  • December 25 - Christmas

Holidays falling on Sunday are observed on the following Monday. Holidays falling on Saturday are observed on the preceding Friday.

How can I get results?

  • Online Access (requires registration and security code)

  • Hard copy mailed by the USPS to veterinarian clinic

  • Fax to veterinarian clinic

  • Pick up paperwork (veterinarian or clinic only)

  • Special courier service (cost provided by veterinarian)

  • Phone call from veterinarian or clinic

How can I obtain scrapie tags?

The National Scrapie Education Program is administered through the USDA Veterinary Services Office in Pickerington. That office can be reached by calling 614.469.5602.

How can I schedule a time to bring and animal in for a necropsy?

Animals can be brought in between 8:00 and 4:00 pm Monday through Friday as long as confirmation is received from the veterinarian's office.
