How do I get forms?Regulatory forms can be requested by a licensed and accredited veterinarian three ways:
Diagnostic forms are available online: ~BACK TO TOP~What is the latest time to drop off a Coggins test and be able to receive results the next day?If the test is submitted by 2:30 pm, the results will be available by noon the following work day.

What are the lab hours?Monday - Friday. 8:00 am to 5:00 pm. Excluding the following holidays:
Holidays falling on Sunday are observed on the following Monday. Holidays falling on Saturday are observed on the preceding Friday.

How can I obtain scrapie tags?The National Scrapie Education Program is administered through the USDA Veterinary Services Office in Pickerington. That office can be reached by calling 614.469.5602.

How can I schedule a time to bring and animal in for a necropsy?Animals can be brought in between 8:00 and 4:00 pm Monday through Friday as long as confirmation is received from the veterinarian's office.
